ORGANIZATIONAL SUMMARY:
Patient navigator component
The patient navigator approach includes five functions: 1) establishing an effective working relationship; 2) encouraging identification and use of strengths, abilities, and assets; 3) supporting client control over goal setting and the search for needed resources; 4) viewing the community as a resource and identifying informal sources of support; and 5) conducting case management as an active community-based activity.
DUTIES:
Patient navigator goals will be to assess and assist participants in four primary domains:
1) Medical: PN will facilitate patients completing antibiotic therapy for their acute infection and will help patients meet other medical needs, such as getting to other medical appointments, managing medications, and encouraging adherence to treatments.
2) Substance use: PNs will use motivational interviewing techniques to ascertain barriers and facilitators to reducing substance use. They will ensure medical treatment for substance use disorders from the medical providers, according to patient desires. They will assist with linking to other community substance use and/or harm reduction programs (residential treatment, intensive outpatient, syringe services programs, etc.). For patients who continue drug use, the PN will promote harm reduction and help facilitate safer drug use.
3) Housing: For participants experiencing homelessness, PNs will actively connect participants with local shelters and housing programs and food resources.
4) Access to medical care: PNs will help patients obtain access to health insurance and other funding sources for ongoing care, especially in the post-intervention period.
QUALIFICATIONS
Minimum Qualifications:
This position requires a High School diploma or equivalent and 1 year of experience.
