About the role The RN-Hospice Liaison expands our visibility and referral base in the community and among health care agencies by; education, acting as a resource person and representing the Hospice Program during referral, intake, assessments, and discharge planning etc. Participates in outreach services to further the community's awareness of hospice services through in-services and meetings. Actively participates in providing the community information of both hospice care in the inpatient and home care setting. Essential Job Functions Improves patient access to Hospice.Expedites referral/admission process.Meets regularly with UR/Discharge Planners, Medical Social Workers, Physicians, and Registered Nurses to evaluate potential Hospice candidates.Through the referral/intake process, meets with patents and families to discuss Hospice services.Markets/presents to the health care community/public that variety of services provided by St. Francis Hospice.Provides education to physicians, staff and families for specific subjects related to Hospice. Coordinates nursing service with other disciplines including social services, pastoral/spiritual care, and volunteer services when applicable.Ensures that the Interdisciplinary Plan of Care is kept current/updated of any changes and that the physician is updated on a regular basis as changes occur when appropriate.Teaches, supervises and counsels the patient and family members regarding the nursing care needs and other related problems of the patient when appropriate.PERFORMS OTHER RELATED DUTIES AS NEEDED
