Community Health Worker (Enterprise) (Information Technology)

Base Pay: $21.00 - $31.50 / hour
JOB SUMMARY
The Community Health Worker (CHW) is responsible for helping patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW supports providers and the Case Managers through an integrated approach to care management and community outreach. As a priority, activities will promote, maintain, and improve the health of patients and their family. CHW provides social support and informal counseling, advocates for individuals and community health needs, and provides services such as basic first aid, blood pressure screenings, and simple wound care.
JOB ROLES AND RESPONSIBILITIES
The roles and activities of community health workers (CHWs) are tailored to meet the unique needs of the communities they serve. A CHW's role depends on factors such their education, training, lived experience, and experience working with specific populations. CHWs may perform the following roles:

Create connections between vulnerable populations and healthcare providers
Help patients navigate healthcare and social service systems
Manage care and care transitions for vulnerable populations
Reduce social isolation among patients
Determine eligibility and enroll individuals in health insurance plans
Ensure cultural competence among healthcare providers serving vulnerable populations
Educate healthcare providers and stakeholders about community health needs
Provide culturally appropriate health education on topics related to chronic disease prevention, physical activity, and nutrition
Create Health Education materials and promote health education through various means
Advocate for underserved individuals or communities to receive services and resources to address health needs
Collect data and relay information to stakeholders to inform programs and policies
Provide informal counseling, health screenings, and referrals
Build community capacity to address health issues
Address social determinants of health

KNOWLEDGE, SKILLS, AND ABILITIES

Effective oral and written communication skills
Effective group facilitation skills
Ability to problem solve with shifting priorities
Detail oriented
Proficient in Outlook, Microsoft programs and virtual meeting platforms (i.e., zoom, teams etc.)
Experience in general office functions, preferably in a health care setting
Accurate record keeping techniques
Organization and time management skills

EDUCATION & EXPERIENCE

Violence Prevention Certification or relevant/equivalent work, education or training
CHW Certification within 18 months of hire
Participate in relevant continuing education programs related to position (6 hours annually)

PREFERRED QUALIFICATIONS

Associates degree in health care related field
