JOB SUMMARY The RN Case Manager will provide medical case management services for HIV patients enrolled in the Ryan White Part C, EIS program. Works collaboratively with the EIS team of medical providers, RN Program Coordinator and other clinical staff working with these patients. Responsible in making appropriate community referrals for support services, develop and monitor treatment plan through assessment and progress reports. Responsible for case documentation in the electronic health record for all patient contact, as well as any change in status which requires treatment intervention.
JOB DUTIES AND RESPONSIBILITIES · Developing and tracking individual treatment plans · Coordinating with Primary Care Provider (PCP) and HIV Specialist when changes in treatment plans are necessary and educating patient to understand these changes · Utilizing the nursing process to address patient needs and to effectively evaluate and change individualized treatment plans · Monitoring labs results, reporting to/or consulting with providers and informing patients of results · Monitoring adherence to medication regimes and supporting compliance with overall treatment plans · Triaging HIV patient calls, walk-ins and patient care needs at time of visit if requested by PCP · Liaising with Public Health to inform of reportable diseases and ensure partner treatment · Providing chart reviews to PCP and/or HIV Specialist to ensure HIV specific primary care needs for patients are being addressed · Understanding and application of current HIV care guidelines and evidence-based interventions · Engaging in medical consult meetings with HIV team for medically challenging cases · Facilitating consultation with specialists for medically challenging cases · Facilitating patient involvement in clinical trials · Assisting hospice and other agencies with end-of-life issues · Client teaching and advocacy · Facilitating linkage with social service case managers, hospital discharge planners and Ryan White Part B case managers · Maintaining necessary demographic and clinical information for HIV patient panel · Providing administrative staff with grant related information · Supporting EIS RN Coordinator in programmatic planning · Participating in monthly case management meetings with Ryan White Part B case managers · Planning, initiating and evaluating system change interventions to improve clinical outcomes · Other duties as assign
KKNOWLEDGE, SKILLS, AND ABILITIES · Knowledge of HIV Disease and sensitivity towards issues associated with stigma, diversity and treatment adherence · Ability to work autonomously and seek out consultation from peers and others when needed · Can express the mission of SCHC to other staff, patients, and to broader community. · Knowledge of adult learning theories and group dynamics · Excellent verbal and written communication skills · Computer literacy · Typing 35 WPM or higher EDUCATION & EXPERIENCE · Completion of an accredited RN program · Current California RN license upon hire and maintain current · Current CPR/BLS certification upon hire and maintain current PREFERRED · Minimum of one year's experience in acute care nursing · Previous experience working in an outpatient clinic or doctor's office · Typing 45 WPM or greater
